Customer Service Agreement
Introduction This Agreement outlines the terms for services provided by Dr. Cam Consulting, LLC ("Provider") to the client ("Client"). By scheduling a session, the Client agrees to these terms.
Services Provided The Provider will offer personalized coaching tailored to the Client's needs. This session is not a substitute for emergency intervention. In a crisis, contact emergency services immediately (call 988 in the U.S. or go to the nearest emergency room).
Scheduling and Appointments Sessions are 50 minutes long. The Client must schedule and arrive on time. Cancellations or rescheduling require at least 24 hours' notice; otherwise, the session may be forfeited.
Payment Terms All sales are final and non-refundable.
Confidentiality Information shared during sessions will be kept confidential, with exceptions for legal requirements or immediate risk. Sessions are not recorded unless requested in writing by the Client.
Responsibilities The Provider will offer professional and empathetic service. The Client agrees to participate actively, provide accurate information, and complete assigned tasks.
Liability The Provider is not liable for any decisions or actions taken by the Client based on session advice or for any resulting adverse outcomes. The Client is responsible for their own mental health and well-being.
Governing Law This Agreement is governed by the laws of Virginia.
